First Visit-Private Treatment

Please have the forms below filled out prior to scheduled appointment or arrive 20 minutes early. The first visit includes a new patient consultation and treatment and will take roughly 90 minutes. During the consultation, we will ask many questions, some of which may not seem related to the chief complaint. These questions enable us to gain a clear picture of health status along with the Chinese Medicine pattern of disharmony behind the complaint. We will look at the tongue and check pulses, both of which are valuable diagnostic tools in Chinese Medicine. A full treatment will be done, so please dress in loose fitting clothing that can be pulled above knees and elbows and eat a snack or light meal before your appointment. Herbal and dietary recommendations will be given as appropriate during the first visit.

Payment is due at time of service. We are not in network with any insurance companies. Health Savings and Flex Spending accounts may be used to pay for acupuncture. If you have insurance that covers out of network acupuncture, we can provide a superbill that would have the needed codes for you to possibly be reimbursed.

Follow Up Visits

Follow up appointments take about 60 minutes. A short intake will be done to assess progress and will guide the treatment tool(s) used for the day. A treatment plan will be created based on patient feedback to best achieve wellness goals. Please dress in loose fitting clothing that can be pulled above knees and elbows and eat something before the appointment.
